I think you will need to contact the company who blocked the phone, you need the puk code to unlock the sim card if the pin code is entered wrong 3 times, if its blocked at another end then the block needs to be released by the company who placed it.

a PIN code is entered wrongly three times, the phone line will be
blocked. Irrespective of the owner, it is assumed that you are not the
owner of the phone line when you enter your pin wrongly.

PUK code comes in. Once you enter the PUK code, your PIN code will be
reset. You can then create a ne PIN code.
To get your PUK code,
if your phone is using a SIM Card, Check the SIM card Pack and you will
see your PUK code there. But if you don't have a SIM card pack, you can
get the PUK code from your service provider.
Call the customer
care line of your service provider using another phone, request for your
PUK code and it will be given to you. Note that your service provider
will ask you some few questions about your line in order to be sure you
are the real owner.
Note that your service provider alone is the
only one that can give you your PUK code because it's only your service
provider that has it.
call up your carrier early in the morning from a land phone with your cell in hand...tell the tech (your carrier is the only one who have the PUK #) the problem he/she will give you on the phone instruction to get u back on track......this will take u 5 - 10 min if you call early morning (avoid hold and waiting time)
PUKs, or personal unblocking keys, are used to unblock a mobile phone which becomes blocked. Mobile phones may become blocked when a PIN (personal identification number) is entered incorrectly on three consecutive occasions. In some instances, the phone may become blocked, which requires a PUK to unblock it, and in other instances, the SIM (Subscriber Identity Module) card itself becomes blocked and again will require a PUK to unblock it.
Mobile phones are also blocked when they are reported stolen and if there is an outstanding payment on the account.
It is important to differentiate between unblocking phones (using a PUK) and unlocking mobile phones (using another type of code). More information on unlocking phones will be available here in the very near future.
NOTE: It is a criminal offence for anyone to attempt to unblock a mobile phone, other than the network provider who put the block on the phone.

Do not enter any code into your phone. The PUK code is a code that your service provider has and not one that you would know or have in your manual. If your an AT&T customer they do provide PUK on their website. If you have entered in a code and the phone is now saying PUK blocked you'll have to go to store to purchase a new SIM card.
Default is always 12345678 then it asks you for new PIN. Enter 0000 and enter 0000 again to confirm but if it says incorrect PUK, please stop there and call vodafone customer care line using a different (friends maybe) phone and ask them for correct PUK. they will give it to you FREE. Be carefull because putting in an invalid PUK code many times can make your simcard permanentlly unusable!!! Regards

your SIM by repeatedly entering the wrong PIN code. If you're asked for
a PUK code, you've had more goes at guessing your SIM card PIN code
than you're allowed. You'll need to phone your network provider
Customer Services for your PUK code to unlock the SIM. For security
reasons, it's only the network operator that can legally provide you
with a PUK code to unlock your SIM.With some phones you will get a cert card with your sim card and it might have the 8 digit PUK on it. if this has been entered 10 times or more incorrectly then you will need a new sim card
