there are only certain lock codes to try as default codes with almost all verizon phones its the last four digits of the phone number programmed to the phone also if that doesnt work try 0000, 1111,1234, 4321, and 9999 if none of those work then you will have to take the phone to a retail location and let them hook it to a flashing station and push the software back into the phone to correct the issue its free takes five mins and its a common error my suggestion once you get it fixed if you change your code formt he default always keep a text message locked to the phone with the code or a dummy contact with it in it so you can look it up if you need to

Either the charger is not working or you got a bad plug inside the phone I would take this to a verizon dealer and have them try some things with it. If this phone has a contract to it I know verizon would repair or replace this phone.
