Everything I am trying to print is coming out blank. I have changed the ink cartridges twice, thinking maybe the cartridge was bad, and also had it do a deep cleaning on the print head. Can you please tell me some other things I can check to try to fix this? Thanks!

1. Did it ever 'properly' print pages....?
2. If so, is there enough ink in the printer? You have be able to tell in the printer's program....
3. If you just changed the ink cartridges, or this is a new install, did you remove the pieces of shipping tape, on the back/bottom of the cartridges?

You actually answered your own question, you said it started fading and came out with lines missing. Looks like and could possibly be that the ink cartridge you are using wasn't as full as you thought it was, and the computer really has no idea, it's just software and i do not recall seeing sensors on the inside of the ink tank to tell the computer how much ink is in the cartridge. Try refilling the ink again or maybe a better idea is to buy a new cartridge since over time the cartridges can get contaminants in them which will block the ink from the inside of the cartridge. Can you do a simple color or b/w copy job? If yes, the ph is OK and you might have to reinstall the drivers (delete before that the old ones).
If you cant see any copy job (blank paper) you should change to original carts and run the manual cleaning procedure twice. Wait a few hours an try to do the printing again. You might have to run the cleaning procedure twice again!
You need to put the test page that was printed into the scanner part of the printer and then press the scan/blue button. This is so the printer can calibrate the new ink. When you have done that the next time you turn on the printer a blank page will not be printed.
When you next change ink you will have to repeat the above procedure.
