If the machine is one ofthe newer variety where it is connected to the PC ...then you can alter your "Auto-Dialer" from your PC.
If it is not a PC connected machine then you will have to go in manually and change the programming on each individual number.
Go to Muratec web site and download(FREE) the operators Manual.
Page 2.1 of the manual details the "Auto-dialer" and page 2.2 explains how to enter or change numbers.

Try MENU,*, w9 to get into Initial Programming then scroll to the "Name "you wish to change
Program, I(capital letter I ), ENTER to access initial programming then again scroll to nam you wish to change.
It could have something to do with the automatic document feeder. There could be something in there that is blocking the paper from feeding in there correctly. It could be the feed tires or the belt as well in the document feeder.
It should be looked at by a technician in your area in case there are some parts that need to be replaced.
What is printed on the top of your received fax's is determined by the sending machine. If the user at the other end does not have name ,date and time programmed into their machine correctly then you will get printed on the top of your received page whatever is in their machine.
