I just don't think the battery is getting the charge due to a faulty piece that is between the Chargers plug and the handsets battery terminals, assuming the charger has been verified as a working one.
Hi, you have got bad cells in your battery and this is preventing it from holding any charge all you need to do is get the battery replaced and this will solve your problem, this is a common problem and usually happens if the battery or phone has got hot, but replacing the battery will solve your problem.

This is an indication of defective battery. Meaning that the battery could no longer hold a charge. Try charging the battery while the phone is off. If you get the same problem, try using another battery (if you could borrow).
You may also contact the manufacturer for second opinion.
When we connect the phone to PC it automatically start charging but after 5 mins it stops its because we do not take any kind of action i.e. if you will send some files to phone or will operate phone from PC it will keep charging otherwise as you have said it will stop.
