SOURCE: The DVD blank disk is
The laser head is covered in dust so the sensor detects a very low return signal. This is interpreted as a bad disk.
The sensor must be cleaned. Open the unit, access the mechanism and do a thorough cleaning inside it, access the laser head and using a can of compressed air blow all the dust on, in and around it - be especially thorough with the space between the lens and the sensor. Finally wipe the lens gently with a q-tip.
If this doesn't completely eliminate the problem then the dust has been baked on the surface of the plastic lens - this is unfixable you will have to replace the laser head, the entire mechanism if you can't find just the head.
