SOURCE: Unblocking My Sim Card
Either you need to put a code in the phone to accept any new sim card or the the phone sounds like it has been reported lost or stolen or the previous sim card had the wrong pin number inputted causing the phone to lock and then the right PUK number was not entered. If you want to run to the expense of buying a serial cable for your phone I can pinpoint where you can get software for samsung phones. Look at my profile at mail me. Hope this helps.

You need to contact your provider and give them the IMEI and serial numbers to verify that you own the phone. They will then email you the code.
