Use this USB drive on another working computer to check if your OS has any problem or not. If the drive is detected and you can access the infos stored in this drive, repair your OS, and if not, go to your nearest service center to avail a full replacement or repair.

It doesn't matter whether it is standing up or laying down, but you should keep it in an open ventilated area to prevent overheating. If this doesn't fix the problem you should contact customer support.
Its probably because of how old it is mine never did that till recently. It will still work just keep a disc inside the xbox and itll be fine. well the flash might be corrupted if u have pluged it recently on some mp3 player like those in the care that work on the radio signal, the electricity circulation in those mashines can broke ur flash drive.
if u want to make sure it is broken, u should go to administrative tools, and choose device managment, then disk managmen, if its note there, then its a 90% broken flash drive.
