Hi as you have lost the card the sim card was attatched to then the only place you can now get the code from is your service provider, the PUK code is the securit code for the sim card and the code is random so its not like the security code where you can use a number generator to get the code the PUK code is totally random and is between 8 and 11 digits long.
so you have 2 options open to you if you need to use the sim card thats asking for the PUK code then all you need to do is call your service provider and give them your phone number and they may ask your name and address but once this is done they will be able to give you the PUK code over the phone for free.
now the other option you have is if you dont need to use the sim card what you can do is get another sim card and you will either get a new puk code or the sim card wont ask for the PUK code.

You can locate your PUK usually it is included in your sim card kit when you purchase it. if you lost it already, this is the last thing we can do. Contact your operator customer care or visit them and ask for your PUK code and they are very glad to help you. they will ask you several questions pertaining to your account to verify if you are the real owner. You can also get it on-line on most operator if they have this service at their website.
Once you receive your PUK code, you will need to enter it into your phone to set a new PIN code.

Note: If you enter the wrong PUK code 10 times in a row, your SIM card will be invalidated, and you will need to purchase a new SIM card. The phone will display the following error: "PUK blocked call operator."
A PUK (PIN Unlock Key) code is required to unlock SIM cards that have become locked following three successive incorrect PIN entries. If your SIM card becomes locked, you will need to get a PUK code from your SIM card Customer Care.
Please call the Customer Care of your Network Operator (sim card provider), they will tell your the code. There's no other way or no means to get the code except from the Service Operator. They might ask your few questions related to the Sim Card, like your name or your address, tell them promptly and they will give you the puk code for your sim card. It's also important to note down the Sim Card Number (number written on the sim card) if in case they ask you the sim number. You will also be assisted on how to disable the security settings.

A lost PIN cannot be retrieved, only the PUK can be discoverd, but you will have to call the phone company.

it seems that your simcard are looking for PUK number. PUK means Pin Unblocking Code..1234 is the default code for your SIm or u have to contact service provider to getting PUK.. another option if you remember the time when you buy your sim card it has a small envelope please check if you still have it. look for a small piece of paper it contains the PUk numbers such as PUK 1 and PUK 2. but if you already lost it we do not have any other chance just to buy a new simcard.
PUK are only related to a simcard of your mobile. the worst problem if you input 3x or 4x of wrong number # of PUK your sim will be permanently block so if you encode 2x already don't try again untill you did not find the PUK number or you will going to change a new sim and all your files save in simcard will be lost be carefull. u need to contact ur service provider, call the customer care, they will ask u for ur sim card number which is printed on the top of sim card and will ask u for some personal information regarding ur number. after getting all information they will provide u ur puk number.and make it sure don't put puk code wrong , if put 10 time wrong then u will loose ur sim card and number u will need to issue new sim card with same number but u will loose ur contacts and messages saved in ur sim card.best of luck
