My guess is that your system is getting too hot. That GPU outputs a bunch of heat and so does the rest of your system. You need to bring down those temperatures. Install as many fans as your chassis will allow. 120mm high RPM fans as exhausts say 2000RPM and slightly slower ones for the intake say 1200RPM.
Check the Nvidia website for the latest drivers, check everyday.
Use a program called Hardware Monitor to check the temperatures.
Also install MSI Afterburner, don't change the clock speeds.
Are you sure that the CPU Cooler is making proper contact with the CPU, check this by using the Temperature monitor software.
Update your version of Windows to the latest service pack and security updates.
Check that the PCI-Express cables are making good contact.
