These are specific to your SIM card and only your network provider can supply these. Its very simple. Call your network provider, explain that you want the PIN2 and PUK2, verify your account details, and they will give you these numbers over the phone itself. Its just a matter of a few minutes.
There is NO other way to obtain a PUK or PIN number.
