If you are really desperate for a tip use a metal box staple, the copper colored ones that always tear your hand open if you don't pull them out.

If the tips your using are not the OEM size, you can always use a small piece of paper clip to act as a adapter reducing the ID so the tips make proper contact.

What is the proper output voltage going into the solder tip?

The voltage is nearly unreadable it is so low, but the current is quite high. If the tip won't heat, loosen the tip nuts and re-tighten.

Weller 8200 series soldering gun sometimes fails to heat; tightening screws and/or gently spreading arms often helps. Is there a permanent fix?

There is constant maintenance needed to keep soldering, the key ones are the tip fastening nuts, and the tip itself. The tip eventually needs sanding and immediate retinning, Clean with damp rag during and after each soldering event, and heavily re tin at end of event.

Does not melt solder

The body contains a transformer, which steps your 120AC down to a lower voltage. The lower voltage runs THROUGH the tip, heating it up. Possibility ONE: Transformer is no good, throw it away. TWO: Not enough current is running through tip, because tip is corroded OR those mounting nuts are corroded, loose, OR somebody put new tip into nuts but neglected to bend both legs of new tip 90 degrees. Look at the barrels, you will see little holes in the sides, tip's legs go into those holes.
