If it was issued from a local bank, you may have to physically appear with adequate ID at the counter to request a change or they may direct you to contact the issuing company directly but somewhere along the line, you will have to be able to prove who you are or else retire the card completely and reapply.
I know too many people who should forget theirs . . .

I do not know where your card is, how would I ever know that?
Phone the people sending the cards, not the insurer. Ask when they sent them and if they should have arrived by now. Check the address they sent them to is correct.

A PUK (PIN Unlock Key) code is required to unlock SIM cards that have become locked following three successive incorrect PIN entries. If your SIM card becomes locked, you will need to get a PUK code from your phone carrier to unlock your SIM card. You can obtain your phone's PUK code from your carrier in one of these ways: Log in to Your Use a Technical Support Chat Contact your carriers Customer Care Get Your Phone's PUK Code Online Go to the Phone/Device page. I returned a laptop computer and forgot to take out my usb receiver. Is there anyway I can purchase another one?

First thing I'd do is call the store that you returned the laptop to and tell them it was still in there. Most stores are pretty good for catching these things.

As far as purchasing a new adapter you have dozens of options. Try to match the speed ie a G or N type card. Thats about the only thing to look at. You can also consider a extension cable that lets you put the adapter away from the laptop a bit. Thats handy but not needed. And you also have the option of getting a PCMCIA type network adapter. These fit into the credit card type slots on the side of the laptop not the USB. These are a bit faster and take less to operate so they don;t use up as much cpu or memory to work (better choice if it is older slower laptop) the only problem is they cost a bit more.
Stick with a name brand and try to find a good warranty. Some of the off brand cards are 30 days and that seems to be how long they last. A good one will last years.

Go to the bank that issued your CREDIT CARD, and ask them to help you set a new PIN (Personal Identification Number) for your card.

Trying to find a replacement mini pci wireless card

That jack is 4-pins of USB and 2-pins for 3.3 volts .  The card is somewhat proprietary. EBay WM168b  it was manufactured by Airvast Tech

If you have a clean record but no history, most banks will issue a card in return for a deposit (typically in the hundreds of dollars) that stays in their hands until the card is retired. For example; one son paid in some $300 in kind of an escrow account and can charge up to that amount before the card bounces but the balance must be brought back to that initial $300 within some grace period.   If you have been paying rent, car loan, utility bills, phone, etc., these can all be used as references unless you have burned them by paying late or not at all.
