The monitor OSD pops-up every time the computer boots. It's a 15" CRT. Video Card is nVidia GeForce 4 MX 440 AGP8X 128Mb.
It started when I downloaded the new driver from the nVidia site and installed it. When I restarted the computer the OSD popped-up and the resolution was very low. I decided to uninstall the new driver and the resolution and display went back to normal, but every time I restart my computer, the monitor OSD keeps on popping-up.

you forgot to say, screen dark? and power light off. does the OSD work? Push button that pops up the OSD screen for bightness, does that work> \can be one of 4 things. 100 % dead. try it on another PC> bad power cord bad video cable. bad LCD bad PC, bad Video card in PC or some game using an illegal video card mode. try another PC and see if the LCD works there. easy huh?
sounds like you need to run a screen config wizard. If you have a true Nvidia driver you can do this. Otherwise, re-download the driver from the main website http://www.nvidia.com/page/home.html
and it'll work for you
That sounds like a monitor issue, not a video card. Gradual fading means components within the monitor are wearing out. However, I strongly advice against attempting repairs.
CRT monitors hold a strong electrical charge. People have been injured and even killed by attempting to work on them without proper tools and training. A 17" CRT monitor is too cheap to be worth the risk (you can often get them used... I got mine for $20 at Goodwill).
To prove which Card or monitor are you in the position of either
A- getting hold of another monitor
B Another computer connected to troublesome monitor
Either way will establish which is the troublesome one.
