If PUK BLOCKED is shown, you had enter your PUK ten times incorrectly.
Your sim card is locked, you will need a new sim card.

You can get your PUK Code from your service provider. Should be a
quick call after they verify your account info. Unfortunately there is
not DEFAULT PUK Code and provider is the only one that can tell you
what your SIM PUK was set to.
