PUK code are unlike security codes that are handset dependent. PUK codes are sim card dependent. You can get the PUK code from the sim package, on the card where you took the sim card. If you dont have this any longer, I suggest that you call your service provider.
PUK code is asked by the SIM/UIM card and not by the phone. It is generally asked by entering wrong PIN code more than three times. So you need to call your network provider's customer care to know the PUK code.

To unblock you sim you will need a PUK code.
Only your Sim issuer (Operator/Provider) can provide you with this as all handsets are different.
Phone up your provider and tell them you blocked your sim and require a PUK code. They will tell you it when you give them the IMEI number of the handset you locked it in. Look under the battery or type *#06# into the handset to find it.

If your handset asks for PUK code, you need to contact the service provider and provide them 20 digit sim number for the PUK code .
Service provider numbers will be toll free.
Once you get the PUK code, enter it . And then it will ask for new PIN number, you need to enter 0000 twice.
