you need to contact with your service provider to whom your sim belongs. they will provide you the PUK code and your phone will be enabled.

well if you still have the envelope n sim holder the card gave with it should have the PUK pin on it.
If it is not on your sim card envelope....you should call your provider to give you the puk unblock code.
