The Evenflo company web site does offer instruction manuals for many Evenflo products:

There is a section on that web page, with the heading Playards & Portable Beds. Perhaps one of the model names listed there is the model name for your Pack and Playard.

The unlocking code for your SIM card can be gotten from the SIM pack at the time of your purchase. Simply locate the SIM pack where you kept it, the unlock PUK numbers you require are there.

The other option is to contact your Mobile network service provider I.e. your blocked/locked SIM network company

Puk blocked on phone Cannot access anything but emergency calls

Get the SIM pack. In the SIM pack, there's a card where you can see the PUK1 and PUK2. You may use one of those codes to unlock the phone.

If you could no longer find the SIM pack, contact the wireless provider to unlock the phone.

Your phone will have to have upgraded firmware to make the switch.
