They make disk repair tools that buff the surface of a disk, they work pretty good. Here's a link to one such device sold on amazon
ur disc readers probly broke/faulty u will have 2 get sony or w/e ur brought it from 2 fix it but this is a way you can play ur games still idk about dvds never tried myself. right if you have a demo go onto that if not download 1 via ps store 1s ur on the demo press the ps button on your controller and go to were the disc icon usally is it should be there now then click on it and say yes 2 quit demo and there u go ur on ur game
It may just be that your dvd drive's laser is dirty (idk how) but thats what was wrong with mine. It would say that there way no disk or that my 260 games were dvds, then I took it apart and cleaned the laser. That fixed my problem so it might work for your system just google "how to take apart 360" and you should find some guides of taking it apart. Anyway hope this helps!
If Wii gives Disc Read Error, first step you need to check is to try
another disc or clean the exisitng disc. This will confirm that problem
is with either Wii or Disc. If the console itself is giving disc read

Have you tried re-installing the Action Replay Code Manager through the
disc? I find that re-installation works for most computer programs when
the first-time-installation fails. Just choose the most recommended
settings (don't do manual stuff) and try again. If it still does not

well ur error might be that ur disc is dirty or scratched... u might not be putting the disc in right ( if it is standing vertical face the disc label to the right... if it is horizontal, the face the label up). idk that is all i can think of.. and u probably r doing thos things right ... i hope this helped
-. Use a standard CD/DVD Cleaner, they look like a normal CD but with two miniature brushes on them. This will only clean the lens. Sometimes this is all you need to be up and running.
-Clean the lens and mirror manually
-Adjust the Potentiometer (pot) on the laser.
