to get your puk code (probably you entered 3 times a pin code that was not the real one ...) you will have to call the provider ...They are willing to help you cause this code is usually written on the plastic card that your Sim was on ...and if you do not have it anymore they will unlock your card or give you the puk code if you manage to identify yourself or let them know the phone number and your name..
