I lost my handset three days ago and i only have the IMEI....is it possible to track it down? and probably, can i identify the SIM card in use with the phone currently?

You will need to contact the carrier and report the phone as lost so that the phone can be flagged. Since you mention thief and police, then you should contact the carrier and report the phone as stolen, they may require a police report filing # as well. The carrier likely won't let you know directly, but likely report the phone status to the police department where report was made.
