It sounds like a fault on the line output stage, but to be perfectly honest I have never seen inside a projection tv so I can only guess a faulty line output transformer or line output transistor. If not it could be some of the related components on the line output stage. Hope this helps.

We have a constant buzzing/whining noise when the TV is on.

Is it a whuring or buzzing sound? When color wheels go bad they can make a lot of mechanical nose, cooling fans can do the same but they do not spin as fast as a color wheel does

The fan intermittenly makes a noise and the color changes on the tv

You need to give model #. Most likely you have a dlp tv and its not the fan but the color wheel that spins at very high speed and creates correct colors on your screen. This color wheel makes noise when its bad and colors go off when its running at slower speeds. Its going to fail sooner or later as its going bad now. Its a professional job to change the color wheel and could cost you $280-320 depending on the model. Fan generally never make anoise that is loud enoug to be heard unless its blades are hitting some obstru ction. Color wheels make whining or in worst case screeching noise when bad. check out www.fixurtvs.com for more FAQs. Thanks if you voted for me.

No screen or sound

there are only two moving parts in your tv a fan and color wheel, that noise is classic of a color wheel, I had the same tv and problem. I replaced it myself but it is a little tricky. part is around $175.

Samsung DLP HL-R4266W..no sound or picture. TV is completely dead

Assuming the noise is a high-pitched whine, then yes, it is the color wheel.  If the color wheel goes out, the TV will turn on for a while, then turn off when it can't establish color wheel sync.  The color wheel has to be in sync with the mirrors on the DLP, so if sync can't be established, the TV turns off.
You can order a new color wheel from Sears or partstore.com (among other places) and replace it yourself, if you are so inclined.
