Question about Cell Phones
If you programmed a phone you had to have used your owners manual to walk you through it.
Next time you do this write the number down in your owners manual.
In the meantime since you are the only one that knew the number you have no choice but to take the phone to your service provider and have them unlock it for you.

After you enter your lock code a fail repeated times, a PIN2 code should pop up. That code is available through your service provider. Once you get the code, you will be able to change your lock code to sometime you may be able to remember. Hope this helps.

SOURCE: forgot my lock code for
Have you tried the default code?
By default the code is the last four digits of the phone number assigned to the phone. However if the code was changed there is no way to retrive it other than taking it to a tech and having the firmware reset.
Hope this helps
