My 1/2hp opener after reaching full open status will slip back down 12 to 16 inches and then re-activate the motor and go back up. This will repeat 3 or 4 times before finally stopping at one or the other position.

Check the micro switch that will be activated when the door reaches the closed position. It might need to be readjusted as it is supposed to be activated just as the door reaches the fully closed position to stop the further movement of the door. If it does not get activated at that point the door will try to open further but sence an "obstruction" and start opening again as a buildt in safety measure for protection against possible injury or damage to property.
The code for 5 flashes is RPM sensor or motor overload. Did you remember to put the black plastic interrupter cup on the end of the motor shaft? Also check the small quick disconnect on the RPM sensor printed circuit board. Make sure it is connected securely.
I'm no expert, but on a previous door opener I had (don't know the brand), the side adjusting screw basically adjusted a limit switch, which was located inside the plastic housing (where the motor is). I was able to remove the housing, figure out which limit switch was actually being contacted "too soon" on the way down and adjusted it while watching it.
Just a suggestion, you might take a look.
just keep your hands out of the way of the chain and sprocket if you do take the housing off. .
Thats one of the symptoms of a lightning strike or a power surge. Without actually being there I would suggest replacing the back-board. But as with lightning strikes and power surges sometimes other electronic components could have been damaged as well.
