Its asking for PUK no. call at your network service provider and ask for PUK for your sim card,they will easily provide you.

If its asking you for a puk code it means you locked your Sim card out of your phone, you need a new sim card OR you can call your service provider and see if they can give you the puk code.

If you have entered the puk code wrong too many times you may have to get a new sim card.
