Most people have purchased a new bike for their child and sometimes run into a problem installing the pedals. Take note on some models made the pedal may have an L and a R on each end of the threaded end of the pedal. That will mean the one with the R is a right hand thread mount. The one with the L is a left hand thread which is install counter clockwise instead. The left side is the side is considered as a car is, drivers side if you know what I mean.
This should help some of you when assembly these sort of pedals

The right foot pedal has fallen off on my model recumbent cycle. The threads of the crank arm seem to be shredded. What do I do?

This can happen if you do not make the pedals tight enough when you first install then. Contact the manufacturer or seller. They should carry the crank arm. Note that it takes a special tool to replace a crank arm. Most bike shops have crank arm pullers for sale. You can contact the manufacturer to see which crank arm puller is required. When you install the new pedal, make it very tight.

I need to replace the right pedal on a Mongoose GMX ladies bicycle which has been chewed off by a dog. I need to buy the pedal and also would like to get an owners/instruction manual as the used bike was given to my wife by a friend.

I tried to find the user manual online but I can get one, I would suggest to visit your manufacturer's website for you to get one.

For replacing a pedal, you just need some tools like open grinch depending on the size of your pedal.

I would like to remove the pedals to teach my child to ride on 2 wheels but cannot find the manual

In my tips and tricks section is a link to the manual. You can also take the bike into any bike shop and generally they are happy to pop the pedals off for you. You need a pedal wrench to remove the pedals, so if you don't own one then this is the simplest and cheapest solution.

gave my 10 year old son a 20" huffy axis this past Wednesday for an early Christmas present. The pedal fell off Friday! How do I put it back on? The pedal, as well as the thing that screws into the bike, came off.

Your pedal falling off is never a good thing, nor is the entire crank arm assembly.

For this one you are best taking it into your local bike shop and have them take a look at why both of these fell off. There are a number of reasons, from stripped thread to missing bolts and nuts to improper assembly. None of these are a good scenario with a childs bike, or any bike for that matter.

Alternative, and my personal suggestion - Take the bike back to where it was purchased and exchange for a new one. These parts should not fall off after years of riding let alone 2 days, and would constitute a warranty and safety issue.
