Hi, if your mobile phone from any network is asking for the PUK code then all you need to do is contact your service provider by either going to their web site or contacting them by phone.
The PUK code is the code for the sim card this isnt the code for your phone the phone usualy asks for this code if you have entered a wrong security code more than 3 times, all you need to do is either navigate their web site under the help section and locate the PUK code for your phone, if how ever you cant locate this code on their web site then all you need to do is give them a call.
This code they will give you for free just remember to write it down when they give it to you in case you ever need it in the future.
The PUK code they give you will only work with the sim card that you are asking the PUK code for, every sim card has its own PUK code.
If you enter the PUK code wrong more than 5 times then your phone will be hard locked and the sim card will then be useless.
If this happens then you need to take your phone into a phone shop and get them to unlock your phone, how ever they will charge a small fee for this service.
