I keep losing my serpentine belt, have replaced the belt and the front idler as directed by my service shop it appears to me that the tensioner is canted could this be my problem all along?

My 1995 gmc safari turns over but won't start. After running the battery down, I hook up to my friends car to jump it. The van then starts right up. As soon as I put it into drive and press on the accelerator the van coughs then stops.
