In Dreamweaver MX I clicked command button and selected Advrs
You may try running a repair of the program from the Control Panel > Applications list. (If you are running Windows.) Basically follow the same steps you would to Uninstall the program. Bring up the Control Panel. Find Programs. Select Uninstall a Program. Click on Dreamweaver. When it is highlight, look toward the top of the window for a "Change" or "Repair" option. If there isn't one. Click "Uninstall" but BE VERY CAREFUL. Once the Uninstall box pops up there may be a radio button (like an option to click) for "Repair" - That should restore any lost files and hopefully resolve the issue. Otherwise, a complete uninstall and reinstall may be warranted. Hope this helps! - Cheers! (See image for example)

This process includes naming the site, placing the local files on a directory on your hard drive, and showing Dreamweaver where those files will be published on the Web. Follow these easy instructions to define a new Web site for Dreamweaver .
First, you should go to the "Site" menu and choose "Manage Sites".
As you can see from the image below, this will open a dialog window. Choose "New Site" from the "New" button.

How to create index.html while designing web page?
If you are using dreamweaver then dreamweaver will do all of that for you - if not then use notepad and when you save it as a txt file close down notepad and ensure that you have file extensions showing ( my computer --> tools --> folder options --> view
hide extensions for known file types ( untick the checkbox ) and apply as per belowwebsite link
then change the file extension from .txt to either .htm or .html
