Not sure I understand the model you are looking for, but what you need to do is get the measurements and search.
This is one URL I found that could help you.
http://www.bizrate.com/lamp-shades-bases/ceiling-fan-shades/
You can also look at the Home Depot or Lowes.
Worst case you can replace all of the globes as opposed to trying to match one or two you already have.
If this information helps you in any way I would appreciate you rating me in the Thumbs Up section of FixYa.
If I can help in any other way, please feel free to contact me.

Got a 52
My local Home Depot stocks these, as does Ace Hardware.
You can also
buy them online if you like (that link was just the first shopping link from googling "e12 candelabra bulbs"
Just be sure to rate my fix as "helpful" before you go shopping! :)

Can you please tell me
Its really easy but you need to be safe, make sure all circuits are off to the outlets you are working on. Usually everything is color coated black goes with black (hot), white goes with white, ground goes with green or copper make sure you use electrical tape to tape up, then assemble from there using istructions. Hope this helps
