You thought it correct. The splitter or the jack filter at the phone jack is blocking the pair. Normal pairing does not have any settings with Phone jack filters.
Please check with AT&T if they have some settings available.

GE 21518 Celluar Gateway won't allow use of touch tone menus.
This is the "nature of the beast" with this unit. I have found the same issue. It seems to vary based on which cell phone you use it with. The Moto Droid I had briefly seemed to allow the DTMF tones to work pretty well. Otherwise, this is a great product. You'll just have to grab the actual phone to enter the tones. :(

There is an undocumented solution to link difficult devices. Instead of pushing ad releasing, hold either button down until you get a FAST BLINK and then try linking again.

Be sure the the headset in in 'Pairing' mode.
When in pairing mode, the light should be flashing red and blue or have a steady blue.
The put most headsets in pairing mode, turn headset OFF, then hold the power button until it goes into pairing mode as discribed above.
Then you can discover it with your phone or PC. Pin (pass key) should be 0000 (4 Zeros) or try 1111, 1234, 9999
How do I set up pairing mode with my bluetooth
Press and hold the power-on button for 10 seconds or until the LED starts blinking. This places the headset in Discoverable (pairing) mode, and you can then search for it from your phone.

if your system knows the headset already and is set as authorised will not prompt for a passcode each time. set your system to unauthorized from your paired device list and reconnect this will force for passcode prompt.
I have a GE 21518 Bluetooth Telephone adapter. If
Caller ID is only sent to devices plugged directly into the "PHONE" jack on the back of the 21518. I'm guessing your TV DVR or Cable Box is connected directly to the real phone line. Maybe you need an RJ-11 splitter so you can plug both your phone AND the cable box in to the "PHONE" output of the 21518. I appologize if you've already tried this.
