From memory I think there is an option to calibrate the Paper Out Sensor, in the Tech Access Menu,
Press the LINE FEED and ENTER buttons for five seconds until the printer flashes up TECH ACCESS on
Then access the menus as described below.
  1. Press the MENU button
  2. Press the DOWN ARROW button until the display reads TEST MENU
  3. Press the DOWN ARROW button until the display reads DIAG
  4. Press ENTER
  5. Press the DOWN ARROW until the display reads CAL-PAPER
  6. Press ENTER
  7. Press the DOWN ARROW until the display reads CALL PAPER PAPER OUT
  8. Press the ENTER button
  9. Follow the Onscreen Instructions
If the above is not in your printer menu look for a PAPER OUT THRESHOLD option (PAPTHR)? Increase this value.

The TECH ACCESS menu should have a METER function which will show voltage change readings for the PAPER OUT Sensor, enter the Meter Mode and select the PAPER OUT option and insert and remove paper to ensure there is at least a 3 volt voltage swing from paper in to paper out.
If you get voltage swing but not 3 Volt adjust the paper out sensor position to see if you can improve this reading.
If there is no voltage swing its likely the paper sensor is faulty.

If you are having problems with recurring Paper Motion Faults that don’t seem to stay
fixed by doing the paper motion sensor calibration, try the following:
· Check the back of the form along the left side to ensure there are no black (dark) areas
in the area of the tractor drive holes. (If there are, this is probably the problem. If not,
proceed on down this list.)
Clean the sensor on the upper left tractor then run the calibration routine again. If it’s
not better yet, continue.
With Tech Access ON, do the following:
Go into DIAG - Meters - Paper Motion and monitor the sensor reading with paper
installed and removed. If the reading with paper installed is in the 300 to 350 mVolt
range, change the sensor (even if it seems to run the calibration alright). A more
normal reading would be something greater than 600 mVolts for the MT691/6092 and
1000mVolts or greater in the 645/661. Don’t forget to recalibrate the sensor. If it’s
not fixed yet, try this next step.
Run a configuration printout and then do the following:
1. Clear the NV Ram (Tech Access feature ON)
2. Calibrate the Paper Motion and Paper Out sensors
3. Restore the configuration based on the print-out you just ran

After removing the ribbon deck there are 2 large Allen screws that hold the shuttle in place after they are removed the shuttle will move tward the platten and lift out.

There are two likely issues. First, install the appropriate drivers for the printer from here... http://www.tallygenicom.com/products/productdrivers.aspx?id=5976. If that doesn't solve the issue, the LPT port may be disabled in the bios of the computer. To solve it, enter the bios and enable the LPT port. (Normally under "Integrated Peripherals"

Try to adjust the "Bar" setting in the Graphics menu. Here you can specify the density of the barcode. I have seen the best results using the Medium setting.

This error indicates that there is most likely an issue with the Hammer Driver's you may need to replace the logic board.
I would however try upgrading the firmware to the latest version, I think this is version 4.0 as I believe this may contain some fixes for Hammer Voltage errors.
