sc495 is a problem with the toner supply system. It may be as simple as a clump of toner clogging up one of the hoses, or it could be a bad toner end sensor or bad motor failing to transport the toner.

Turn off, remove print wheel, turn on, turn off, reinstall print wheel, turn on. Make sure PROPORTIONAL SPACING is not selected.

Two things. Multipart forms have a film on the top copy. Change the ribbon to another brand. Some work better than other. Probably if it printing some on the top copy, and it works on regular paper.
If it does work on regular paper its the ribbon may be old or change to different brand.
A work around is to use eraser on the form where you going to print. This will the remove the film, and allow to type there.

If you have checked the ribbon, and see that it is moving by looking at the letters on it. You may be in stencil postion.
Just below the ribbon there is an r and and s Push the r button and try it.

One it could be a bad ribbon. the other is a bad print hammer, or is failing, there is platic cap on the selenoid that cracks after years.
Needs replaced most likely, Or the selenoid plunger is gummed up.

not sure what you call comb guard, but most IBM Selectric parts are secured in with screws, but sometimes pivot screws from the side, loosen nuts, screw out and see if your "comb guard" comes out. As I say is it on top of keyboard or under the keyboard we might have called it something else many years ago work on these machines.There is quite of IBM machines on ebay,

Nothing you can get around. It needs a new fuser which heats the toner to bond to the paper. You will need a service provider. If you are a business, you should look into service contracts where you pay a per copy service and everything is covered.

Get some switch cleaner from an electrical shop and spray the solenoid that strikes the wheel.

As an antique, repair would be fairly difficult if it needs parts. :>)
