The belt has probably broken and is flopping around inside. You can get a new belt at an Aerus Electrolux store (see below link). Order one if there's not one close.
The biggest problem is getting it put together correctly afterward. The screws need to be replaced in a certain order. Unfortunately, a service manual is not available.
Warranty. If you purchased it from an Aerus representative, there's a two year warranty on the vacuum. It does cover the belt. Just take your sales agreement ot the store and let them replace it. If you've taken the vacuum apart, however, the warranty is void.

No. The only reason the belt should come off is if the tension wasn't tight enough. This could indicate the wrong belt or worn/loose belt. The belt can be tightened by moving the motor away from the brush slightly, then tightening the screws on the motor. There should be long enough slots in the motor to allow for this.
Brush does not rotate
This is due to a broken belt. You can get a new belt at an Aerus (Electrolux) store. If you can't find Aerus in the vacuum or appliance section of the yellow pages, check the below site to find the franchise nearest you.
These can be tricky putting back together, so I recommend letting their service tech do it for you.

Replace belt on Electrolux Epic 2500 SR Upright
1. Start by removing the screws that secure the bottom plate of your vacuum. 2. Once the bottom plate is removed, you will find the belt inside, as well as the belt pulley and drive wheels. 3. Disconnect the belt from the belt pulley. 4. Align the new belt over the belt pulley and drive wheels so that the teeth of the belt match up with the grooves on the pulley. 5. Wrap the belt around the pulley and drive wheels, and then reattach the belt to the pulley with the supplied clips. 6. Reassemble the vacuum and you're done!
