As far as I know there's no device supporting CDMA/GSM yet. Although some GSM phones are tri&quad bands is nothing to do with it. The CDMA is totally different system as GSM. Let say comparing diesel engine with petrol engine.

Is it asking for PUK code?
If thats the case you can call up the customer service and get that code from them.

PIN stands for Personal Identification Number found in a sim pack as yu purchase it,printed in a piece of paper, u should have kept it.
Usually it has "pin1" and "pin2" which is both "1234" but if u had change it,it is surely differnt right now
If u haven't, try to enter the digits 1234. By the way....pin1 and pin2 is accompanied with "puk1" and "puk2". I hope i've supplied the right solution.

I wonder if I told you how I found the fix whether you would use the information for use on your website.
