Hi, Syam this answer is geared for Harley Davidsons but the principal diagnosis is the same you could have a faulty sending unit or gauge, there could be a problem with your wiring. On models with the fuel gauge posing as a left-hand fuel cap the gauge is a press fit do not try to unscrew it, simply ample force lift up in a rocking motion to remove gauge. Disconnect wires to the gauge and with a set of positive and negative jumpers from the battery connect the positive jumper and flash the negative, gauge should peg full, if not replace the gauge. With a test light check for power to the sending unit, then remove the sending unit and manually operate the float arm slowly with the correct wiring hooked up to the fuel gauge and sending unit and the ignition on, the gauge should respond accordingly if not replace the float arm assembly. If your fuel gauge registers the incorrect amount of fuel in the tank you can try bending the float arm the necessary amount for a more accurate reading. SOURCE: fuel meter reads empty, even when tank is full.

Most likely your fuel level sender is stuck or faulty, remove from tank and check. check all electrical contacts for corrosion.

Town and Country Van running out of gas and guage says half a tank what could be the problem?

If your fuel gauge is showing you in accurate readings this is normal to all vehicles that are not used to fill-up gas full regularly but sometimes the cause of the inacuracy is due to faulty float valve in the gas tank you need to replace it

Dash gas gauge stops showing proper gas level once it hits half a tank. Then gauge will fluctuate between empty (light will come on), full and anywhere in between but seems fine from full tank to half

If the trouble with the gauge started after you worked on it then you likely damaged or created a restriction to the movement of the sending unit. You will have to remove the tank and double-check your work.

Fuel gauge shows full, even tank is empty.

Fuel gauge sending unit might of went out. My first guess, is the fuse. Look in your manual, see where fuel gauge fuse is and first replace it, if it just happend , it is mostly likely a fuse or connection issue.
