Try replacing the bolt in the shifter. If that doesn't work, you will have to replace the shifter.

check for broken selector cable, linkage or pivot for the gear lever if direct into the top of the box
in gear selectors that are direct into the top of the box , the regular failure is the pivot pin in the turret comes loose and will not allow effective gear selection

Why does the gear shift lever always come loose?

Hi, Anonymous the splines on the shift shaft may be worn out for this situation I would call my local dealer or reputable shop's service/parts department and inquire about any possible quick fix, answers, or parts inquiry. Shifter broke

Keep your fingers crossed that it is simply a loose pinch bolt on the shift arm. To check, look closely at the shift shaft end as you move the shifter up and down. If the shift shaft is not moving with the shift lever, the pinch bolt may be loose, or more likely, the splines have worn out on the shifter due to a loose bolt. To fix, grab a 10mm wrench and loosen and remove the pass-through bolt that is on the shift lever and replace the shift lever. I provided a link to a used one on eBay for $15. If the splines on yours are good and the bolt is tight with the shift shaft moving properly, then the internal shift fork is damaged which will require a teardown. 91 Kawasaki EL 250 EL250 Eliminator SHIFT LEVER

When I start to peddle the gear shifter moves as if im shifting gears and the chain comes off. Any suggestions?

I'm assuming you have levers on your shifter. If so, typically, the problem is the screw on the side has come loose allowing the lever to move. Tighten the screw a little see if it resolves your problem. If not snug it up some more. The tension of the screw creates the friction to keep the lever from moving.

My 1998 softail custom won't shift above third gear, and i have a really hard time getting it to shift back down

It could be your shifter pawl in the transmission. But, check for any binding in the long linkage between the shifter and transmission. Disconnect the long connector link at the shifter in the front and check for free movement of the connector and the shifter itself. If that moves freely, then I'd suspect the shifter pawl. You'll see the short lever that connects to the shifter pawl shaft. Make sure that it's tight and not loose on the shaft. If it's loose, the splines are probably worn in the lever and on the shaft as well. Move the lever like it would move to shift to a gear. You should feel spring tension against the movement and it should return to "center" when you let go. If not, the shifter pawl return spring is broken. Usually when this happens, all you have is first and second gear.

I'm guessing standard transmission? Think about getting it checked. It sounds like the splines (areas where your gears fit/mesh together inside the transmission housing) may be coming loose or for some reason are not fitting together like they should. Are you having any issues changing gears? Have you noticed any excessive play in your gear shifter? Is any fluid leaking out from the housing? I'm thinking something is loose in the housing itself or you may also get the clutch/throw out bearing checked - sometimes that can cause vibration/rattling or even a whining noise as it's going out.

The shaft that goes into the gear box, behind my primary is loose and not clicking into gear normally. I am told that the shaft may need to come out and be put back in properly, but also was told that...

To check the adjustment of the shifter pawl, first you need to have that lever tight. Here's the problem. You probably are going to find that the lever will not tighten. The splines on the shaft are worn out. This is bad. You must pull everything on the left side of the bike to get to the lever to replace it. If the spines are worn on the shaft, even a new lever is not going to fix the problem, the shaft must be replaced. This is a major job as the guts of the transmission have to come out. The shifter pawl shaft is the one of the first parts put into the transmission and one of the last to come out.

To check the adjustemtment of the shifter pawl, with the bike NOT running and the transmission in neutral, gently push down on the shfiter lever until the transmission tries to shift. Notice the distance. Now do in the other direction, pull up like you're going to second gear. Notice how far the shifter goes before it is stopped by the shifter fork trying to shift the transmission. There should be about the same travel up as three is down and the spring tension should be the same in both directions as well. To adjust the shifter pawl requires special tools as well.

Good luck
Steve

Gear shifter locked in forward gear

If the gear lever is loose,it's the gear connection rod that you disconnect to do a g/box drop.If its not loose, drop the g/box off the leg and the first thing you find might be the problem.A loose bolt,a jammed shifter. It will probably be a connection problem than a g/box problemHope this helps

Gear shifter adjustment

The VZ 800 ,has a linkage system ,it can be adjsted to suit your needs .Firstly , your linkage attaches to the front foot lever.on the other end the linkage attaches to the actual gear box.Its this part that you need to adjust ,Undo the bolt that holds the linkage bracket to the gearbox spline. slide the bracket of the spline .undo all the adjusting screws on the linkage rod ,position the gearbox linkage unit to the required angle you need to position the foot lever to the popsition you want and the adjust the linkage rod to suit the distance between the garebox liinkage unit and foot change.
