My 2000 ford focus does good, til i change into 2nd gear, then from 2nd through 5th gears, it jerks and barely runs. and when it gets real bad, it causes the clutch to slip, what could be the cause of that? it started doing this a couple days ago, one minute the car is perfectly fine, then all of a sudden, like i said, it'll start jerking really hard, and continuously wants to die, and when it gets worse, the clutch slip very badly.

More than likely it's not the linkage. Sounds like the fourth to fifth synchronizer is very badly worn, which will cause the shifter to "pop out" of fifth gear.
