Last time I fixed this problem, I swapped the plugs for those two sensors. That is what that code is. When the sensors where replaced the plugs were swapped.

so after all that replace all the ho2s and o2s sensors using the correct tools so that the wiring is not damaged and check the wiring back to the ECM for burns and rub marks.

Suburban oxygen sensor

The reason for insufficient signal switching can be melted wires. Faulty heater circuit fuse can be the issue. Also if the wire comes in contact with water, the wire gets damaged. Check the faults and go for repairing and replacing the faults.

1996 GRAND CHEROKEE CODE #P0141 AND P0138

Hi there:
Both codes refer at O2 sensor...

DTC P0141 - O2 Sensor Heater Circuit Malfunction (Bank 1, Sensor 2)
This code means that the heated circuit in the oxygen sensor on bank 1 decreases time needed to enter closed loop. As the O2 heater reaches operating temperature, the oxygen sensor responds by switching according to oxygen content of the exhaust surrounding it. The ECM tracks how long it takes for the oxygen sensor to begin switching. It the ECM determines (based on coolant temp) that too much time elapsed before the oxygen sensor began operating properly, it will set P0141. See also: P0135 (Bank 1, Sensor 1).

You will likely notice poor fuel economy the illumination of the Check Engine Light.

A code P0141 may mean that one or more of the following has happened:
open or short to ground in the wiring harness
O2 heater circuit wiring high resistance
O2 heater element resistance is high
Internal short or open in the heater element
Note: Typically a failed catalytic converter does not cause this code. You're more likely to see a P0420 code for a failed converter.

Possible Solutions:
Replace oxygen sensor (cannot repair open or short that occurs internally to sensor)
Repair short or open or high resistance in wiring harness or harness connectors

DTC P0138 - O2 Oxygen Sensor Circuit High Voltage (Bank1, Sensor2)
The Heated Oxygen Sensor (2) located rear of catalytic converter produces an output signal relative to oxygen storage capacity of catalytic converter. Ho2S 2 signal is less active than signal produced by front oxygen sensor. This code sets when HO2 Sensor voltage is greater than 999 mV for more than 2 minutes (time depends on model. Could be as high as 4 minutes)

There may be no noticeable symptoms except for MIL illumination. Possible high fuel pressure can over rich the system.

A code P0138 may mean that one or more of the following has happened:
Faulty O2 sensor
Short to battery voltage in O2 sensor signal circuit
High fuel pressure (not as likely)

Here are some potential solutions:
Replace O2 sensor
Repair short to battery voltage in o2 sensor signal circuit.

Hope this helps; also keep in mind that your feedback is important and I'll appreciate your time and consideration if you leave some testimonial comment about this answer.

Thank you for using Fixya, and have a nice day.

Check engine light

P0041 Downstream Oxygen Sensors Swapped From Bank To Bank
How does a P0041 code trigger the check engine light? This code will trigger the light to come on under the following condition:

The Heated Oxygen Sensor Monitor, a strategy built into the computer, determines if the Heated Oxygen Sensor (HO2S) signal response for a change in fuel trim corresponds to the correct engine bank. If a response from the Oxygen Sensor being tested is not indicated, a trouble code will be set and the MIL (Malfunction Indicator Lamp) will illuminate.
Possible Causes:
Crossed Heated Oxygen Sensor harness connectors
Wiring swapped from one Oxygen Sensor connector to the other
Wiring changed at the computer connection or in the related harness

Checking the fault codes

Yes.  Next to the ash tray there is a black slide off plate that reveals the OBD I plug.  {1996 and newer can be read with an OBD II reader (read for free at Advance auto or Auto Zone)}  Once you reveal this plug on the 1995, you can plug in a single jumper wire (or even a paper clip) in the #4 and #15 pin hole.  There are two rows of pin holes, count from the upper right to the #4, then on to #15 (which will be the second to last pin hole).  Turn the ignition switch on (motor not running), insert the jumper wire for about 3 to 5 seconds, then remove the wire.  The ENGINE LIGHT will flash a hold, then the code, which will go something like this, flash, flash, PAUSE, flash, flash, PAUSE, flash, PAUSE, flash, flash, flash, flash.  Then it will repeat the codes if you just sit there and observe it.  The code in my example would be a 2214 code.
Below are some code I found on a website.  I hope this helps.  Now I ask that you take the time to answer someone elses question.  Lets all share.  Good luck, "Peace through power and knowledge"
