I would check the spark plugs and wires or coil packs. Sounds like the ignition system is arcing.

It sounds like one side of your rear brakes is malfunctioning...or sticking and them braking loose when you move a couple feet.

Everytime I come to a stop I hear a clunk and when I accelerate I hear it again. I can see the shifter moving but I've looked under the truck and nothing is missing or loose. Any ideas?

I would be taking a very close look at the transmission mounts. Two pieces of steel with a block of rubber between them. Over time the rubber will tear or separate from the steel. It's job is to dampen the movement of the drivetrain as well as hold the trann in place. Hope this helps.

When slowing down the transmission feels as if it is jerking in to the next gear.

after checking fluid level is correct, try this, to see if it improves...
into drive, let car accelerate without any gas(do not touch pedal)to max speed (about 30/35mph).stop,then with 1/3rd gas pedal, accelerate to max speed(do not move foot)stop, then foot hard to floor, upto about 60mph,then stop.then only using a max of 1/3rd gas pedal, drive for about 15miles.you have just reset the drivetrain computer setings....the car is fitted with a computer controlled fuzzy logic gearbox,it will try to make you drive as eco as poss, but if used harshly the gearchange can be harsh up and down.....
if driven gently 4th will engage at 43 mph, with a slight clunk.

Clunk sound when I push gas pedal to fast. Is this a drivetrain or transmisson problem?

I would guess that its a U joint in the drive train. When you accelerate slowly it can handle the stress and doesent clunk but when you step on it it clunks hard. Probably the drive shaft U joint, have it checked out but a U joint shouldnt be thw most expensive of the possibilities and would be a good place to start.

Clunking sound at rear axle and not spinning correctly

Hey guys, that clunk that you are hearing is not the differential. all z71s or 4x4 have this problem. its the yoke that enters the transmission on the rear just sand it down with 220 grid sand paper on the outside and inside, the splines tend to get surface rust alot. after you do that put some hi temp grease inside the yoke install it and there you go. the noise stops

Your trouble is most likely not in the transfer case but in the front differential engagement solenoid.
