Check the vacuum line going to the brake power booster, and check valve that goes into the booster (round thing behind the master cylinder). It sounds like you have a problem with vacuum, or the power booster may have gone bad.

Brake pedal hard halfway hard to stop

Not sure why you have scanner readout, but the most likely culprit in the hard brake pedal/hard to stop category would be a faulty power brake booster.

1992 dodge grand caravan with ABS has a hard brake pedal and hard to stop, it makes funny chatering sound from the hyd control moduel, how can I tell if the modual or the abs motor are bad

A "hard brake pedal" is a sign that the brake booster is not functioning correctly. check vacuum line to booster first, for splits or cracks. if none the master cylinder may be bad.

My brake pedal is hard to push in. the car wont/ is hard to stop. i replace the brake pads and the calipers. what could be the problem now?

i guess your brake booster is the problem. try this.... engine not running just pump the brake at least 25 times at the 25 count, just keep your feet on the brake pedal and start the engine. if the brake pedal won't go down even a little... your brake booster is gone. or something with the vacuum.
