Probably a shot seatbelt deal.
You'll have to replace the whole seatbelt.

I have the same problem on my 2004 sienna. I was told to diassemble side panels and get to where the twist is. Then take a regular clothes iron and starch and iron out the twist. Have not tryed this yet, going to today.
