Noises are tough, but try getting under the car when the engine is cold and it has been sitting for a while and hit (with your fist) the catalytic converter-the substrate in cats break up and cause rattles that are especially noticable when the vehicle is running at low speeds.

well I AM trying to help, and the ONE thumb is really nice of you... like I said, noises are REAL tough to diagnose over the INTERNET, maybe you could use your cellphone and post it on youtube...go onto forums for Aleros, there are lots of them, and the people on those forums might know exactly what your issue might be (google "Alero forum')-maybe a better description like it does/does not seem to have anything to do with the engine, or trans, or brakes...like it might be a suspension issue (does it when it hits potholes or bumps)...thanks again for the ONE thumb, though, good luck with your 'rattle'...

Most likely, you have a timing chain that is extremely loose and hitting the inside of the timing cover. There is probably a broken timing chain guide or tensioner. I would not operate the engine if this is the case. If the chain comes completely off, it will destroy the entire engine. It can also knock holes in the timing cover and make the engine lose all of its oil...also resulting in the complete destruction of the engine.

Suspect 99 malibu broken timing belt

If its a 99 4 cyl 2.4 ltr it should have a timing chain. Check your PCV (positive Crankcase Ventilation) Valve. it should rattle when you shake it. sometimes you can clean it with carb cleaner. but for what they cost, just go buy a new one. When they plug up it doesn't let your crankcase breathe and you will start getting oil in your throttle body or carb. Gas engines don't like to run on oil for the combustion process.4 cyl engines dont have a pcv valve just the v6 motor...

99 A4 has very little power above 3K and idle bounces & rattles

sounds like a clogged exhaust system , from the outside of the car does the rattle sound like it is coming from the convertor or muffler? if so run an exhaust system back pressure test via a oxygen sensor port or drop the exhaust and run car with open exhaust and see if the power comes back. doing this will likely set a dtc so be prepared to clear codes after test.

Engine making ticking sound when hot

the internal combustion of the main valve is most likely either clogged or it has a loose spark plug that is constantly providing a spark without oil. Either that or your break pads have gone bad or just some seats and stuff loose, I'd have it checked on by your nearest professional.
