Does it down shift appropriately when accelerating? Normally when you stomp on the gas while in a high gear (travelling at 40mph), the transmission will downshift to a lower gear giving the car much more torque. This can cause the car to jolt or jerk forward and accelerate suddenly.

Make sure the engine is running right, sometimes they wont shift if the engine is missing or losing vacuum. Sounds like the computer system is sensing there is not enough torque to shift into the higher gears so it is preventing it. This could be caused by your engine not running right under load, bad gas, bad plugs or something that is causing your engine to miss under load
soundws like your on the right track change the filter if it still does it run a fuel pump volumn and pressure test? a scanner works well to help diagnosis these problems so get it scanned anyway some mechanical problems may not be far enough out of range to alert the ecm but enough to cause drivability issues?
first of all you need to fix the motor mounts drive shaft hangers and the lower control arm bushings and if you still have a transmission problem . it is in the valve body in the transmission you need to bring it to a good transmission shopas this will be a major tranny overhaul
