This set of symptoms is classic evidence of a stuck open EGR valve or malfunctioning EGR circuit. Keep in mind that a stuck open EGR valve may or may not set a diagnostic code in the computer.
The EGR valve is very easy to remove and clean which should remedy your issue and is generally good preventative maitenence practice. It is situated on a runner which goes from the air intake to the exhaust pipe/manifold.
