Question about 2004 Honda Civic
This will fix it. I just did mine. You do NOT need to pull out your stero to get the Serial Number.
If you lost the code, you may retrieve it from https://radio-navicode.honda.com/

Every car has a different code. You should have gotten it from the dealer when the car was purchased. If you don't have it, try taking it in to the dealer and see if they can get it for you.

Press "Power,' "1," and "6" buttons at same time and hold. A code will display and then possibly a second code will display. Write down code(s) and call dealer with this and vehicle identification number. They can look up actual radio code to use. Alternately, the radio code may be on a sticker in the car somewhere. Sometimes radio is "locked up" from several code tries. If so, disconnect and then reconnect battery before either trying the steps above or just entering radio code, if known.
