I have a similar problem. Only happens after it has been running for a while and only after accelerating from idle speed. was told by the dealer and 2 muffler shops that it was coming from inside the muffler and it had to be replaced. Quoted cost for muffler (part only) is 700.00! Not sure why so expensive.

You just might not hear the noise when it is at speed or the engine vibrations are not at the right frequency to cause the resonance.
I'd check the heat shield around the catalytic convertor; they are notorious for doing this and we have one rattling currently too!

What is causing rattling grinding noise at idle especially in my 89 4.5 DeVille?

The technique I always used to identify where such a noise was coming from was to use a very long screwdriver (2 feet long), while the engine is running. CAREFULLY touch the screwdriver end to each pulley or other object under the hood, and put your ear to the handle. The device making the noise will become very clear.

It could be AC pump/clutch, alternator, water pump, power steering pump, etc.

Sounds like a bad spark plug, spark plug wire. Probably the spark plug wire. They usually act that way when load is applied. It may misfire under light load and not at idle. Try plugs and wires.

Vibration

It absolutely can be, but anyway you should hear that noise when accelerating the engine from idle dont need to drive, just press gas pedal from idle and if there is a strange noise in exhaust then the honeycomb will be loose

Rattling noise coming from the driver's side front end

it could be a heat sheild on the exsaust its not a real problem, but if bothers you that much you could tighten the bolts or even take it off. and about your check engin light it could be your gas cap not being tightened enough because on subaru cars the gas caps must click atlease 4 times or a check engin light will show up due to a code sent to the computer saying the gad cap is loose

Jeep 360 rattle sound at idle in park

it could be a wrist pin, and im not sure if those amc's had hydraulic lifters but sometimes with old lifters when the oil pressure is lower at an idle the lifters arnt filling up all the way so they develope a little valve lash, and when the rpms are raised the oil pressure raises and fills the lifters. you could try chasing the noise down with an automotive stethoscope.
