I am not sure how youarrived at the shift cylinder being bad but I have made it a habit that before doing major work like that of checking all the ground wires for good contact. Alot of Cars now adays depend on electrical for them to work. this has saved several Transmission changes for friends of mine. and is usually a good place to start as it is easy and free.

Your vehicle is (obviously) equiped with a manual transmission. I understand that you have been refilling your clutch resevoir, well the problem should be a leaking "slave cylinder" that is located down on the drivers side of the transmission bell housing. (If you are not mechanically inclined, and do not have the proper tools,I suggest that you take the vehicle to a mechanic to have this procedure performed.) You can purchase a new slave cylinder at your local auto parts store. You'll need to remove the old slave cylinder, There are only a couple of bolts, and one hydraulic line that attach this to the bell housing, which you'll need a "line wrench" to remove and re-install with. After removing the old part,and installing the new one, you'll need to re-fill the resevoir with the approved brake fluid,and then bleed the air from the from the system. To do this, you'll need another person to manually pump the clutch pedal from the driver seat. Once all of the air is out of the system, you should have a good pedal. Again, if you haven't done this before, I suggest that you take the vehicle to an experienced machanic.

Have your transmission oil filter changed;
it is located under the transmission pan
The oil is what makes the transmission shift
When you shut off the engine it stops the transmission pump then when you start up again the transmission shifts until the filter clogs again

1st and 2nd shift solenoids has sediment causing them to stick.or you got damage leaking seals. it might help if you put a little transmedic in transmission dont over fill. if your valve body full of sediment it causing valve in valve body to stick.this might help still might have to go in transmission.

1992 f150 will not shift out of first gear

If the shift lever is floppy and not shifting, drop the steering column so the steering wheel is resting
on the seat. you will see on the shift linkage, a pot metal casting....kind of tube shaped, about
a foot long. It is mounted to the top of the steering column. check for a fatigue fracture on that tube.
if you find that it is broken, it's not too bad a job to remove it. will need a screwdriver with
torx head bits, and a 1x4 inch socket.

Gear shifting is hard to impossible

Synchronize the gears is what a trans does when you shift it, the part that makes this possiable is a part called a sycronizer ring, if they (4 of them) wear out prematurely then you end up with a hard shift issue, does this answer your question? I do assume your clutch is ok?
