Lower hole in the door jamb loosten allen screw until the cap comes off. Upper hole there is a #20 torx slightly down and inboard of the hole, you need a really long torx bit and pull the handle out slightly. It also helps to pull weatherstrip loose so you can see it.

I have the same problem on a Toyota Tercel 1997. I am trying to remove the door panel with the door closed (I can not open the door because the door knob is stuck in the lock position). I think I will have to cut through the panel. Have you found something ? My e-mail is [email protected]

there are special dealer tools required for this and would be very difficult with out them you must obtain a t 20 torx screw driver up to 8 in + to release the handle, open the door and at he latched side you will see black plastic body plugs use a flashlight to see inside, move handle back and forth to see screw operation

There is two bolts that hold the window slider thing in place. One just below one of the door handle nuts inside the door and the other is on the outside of the door (on the inside of the car). Once these are removed it enables you to move that mech out of the way to get to the other bolt.
I undone the 3 latch screws to give me a bit more movement in the rod that connects to the handle too.
This should give you enough room to get to that other nut and clip the rod into the handle once installed.
Once handle is installed simply just slide the window mech back up into position and bolt back in.
