Question about 1992 Toyota 4Runner
There are a few things to check --
check the thermostat and make sure that it is opening properly.
check the water pump belt tension.
make sure the water to antifreeze ratio is correct.
back flush the cooling system.
these are the the things that you will want to check first,, ( starting with the thermostat ). if after these are checked and if everything is good, then there is a chance that the fan is not cooling the radiator properly and there also could be blockage in the radiator itself.
